In order to ease the admissions process, our establishment suggests that the various admissions and insurance formalities be prepared several days in advance. 

The day you come to complete the necessary paperwork, please bring along all documents that your particular situation requires.
Upon admission a down payment may be requested for fees not covered by French social security or private insurance, if applicable.

Documents to provide depending on personal situation:
Certain patient information collected on medical documents is saved on computer file.  Pursuant to the French laws of January 17, 1978, and July 31, 1991, patients may request this administrative data from their physician. 

The medical files from your hospitalization may be provided to you in accordance with French law of March 4, 2002, and upon written request including a check in the amount of 35 €.  
Unless the patient indicates otherwise, his or her family will be informed as to their condition.

For legitimate reasons, and in accordance with the conditions provided for by article 25 of the January 6, 1978 law, you may refuse the collection and saving of personal data concerning you.  The information is provided to the doctor in charge of medical information and is protected by physician patient confidentiality.

International Patients
